Exit interview that surfaces truth

The 6 questions a leaving employee will answer honestly.

What you need: The name of the person leaving and their role. Why they're leaving (stated reason). How long they were on the team. Your relationship with them (you hired them, you inherited them, you were friends, etc.). Whether you're their direct manager or someone else is. 5 minutes of honesty about what you're actually afraid to hear.

How to use: Paste into any fresh AI chat — or into your saved Project / Custom GPT / Gem if you've built your business profile. The AI will interview you across 5 questions to calibrate the script to YOUR situation, then produce a 1-page exit-interview script: the verbatim frame you read out loud, the 6 questions in order (each with a "listen for" cue and a sharpening follow-up), the listening rules to print and fold in your pocket, the one-action close, and the 30-day follow-up text template. Print the script. Run it 2-3 days before their last day. Not on the last day.
